Children’s Membership
Plan Price: $99 / year
Best for: Ages 12 & under without access to dental insurance
Includes: All services included in the adult membership, as well as two fluoride cleanings
Periodontal Membership
Plan Price: $295 / year
Best for: Patients diagnosed with periodontal (gum) disease
Includes: 3-4 periodontal maintenance cleanings, 2 exams, 1 set of bitewing x-rays, 1 panoramic x-ray, and 1 intra-oral digital scan and photos

How do I sign up for a membership?
Let one of our staff members know you’d like to be a member during your next appointment. You can book a visit online here.
How do I pay for my membership?
The membership is a one-time yearly fee. You can pay by credit card or cash.
Who can I add to my membership?
Dental memberships are charged per individual. We currently do not offer family plans, but you can purchase an individual plan for as many family members as you would like to include.
When does my membership take effect?
Your membership is effective as soon as you you’re enrolled at your next appointment!
I’m not sure if a service I need is covered by my membership. What should I do?
Call a Corner Clinic location or send an email to membership@cornerclinic.com. We can let you know if a service is covered or how much it’ll cost if it’s not.
Is a Corner Clinic membership the same as dental insurance?
This membership is NOT the same as health insurance. You will only have coverage for care at our Corner Clinic locations. You will need to sign up for health insurance if you want help paying for care at hospitals, specialist facilities, pharmacies, etc.
